Activity Daily Living
Supervision:
- Meal Time: Supervision of patient at meals time, Verbal cueing for patient to eat
or/and drink fluid.
- Toilet: Caregivers will stay outside of bathroom when patient is inside, but will not
give physical assistance.
- Caregivers will lay out clothes and patient will performs task.
- Bathing & Bowel: Verbal cueing to get the patient to walk to shower or tub
Limited Assistance:
- Meal Time: Caregiver(s) will be physically redirect to eating area, will orient a blind
patient to position of food items on table or tray.
- Toilet: Caregiver(s) will hand patient bedpan, urinal or other stuff but He or she
uses it themselves. Caregivers will stay in room with patient to assure safety
- Bathing & Bowel: Patient is highly involved in care, needs assistance with combing
or shaving. Caregiver(s) get patient in and out of shower or tub or shower chair.
Extensive Assistance:
- Meal Time: Caregivers will guide Patient's hand to mouth for some bites of food ,
but not for all and will put utensils or liquids in resident hands.
- Toilet: Caregivers will help with some clothing before or after toileting and will
assist patient to get on and off toilet or bedpan and diaper.
- Bathing & Bowel: Caregivers will wash one or more parts of patient's body.
Total Dependence:
- Meal Time - Toilet - Bathing & Bowel: Caregivers do everything for the patient.
